Most health insurances do not cover infertility. Conversely, many insurers will provide coverage for diagnostic testing. You may be able to obtain some reimbursement for your care. Your receipt for payment will contain the standard diagnosis, medical, and surgical treatment codes used by the majority of health insurers.

Payment for medical, lab, and diagnostic services is due at the time of service. If you have a managed care contract, you can expect to pay the deductible or co-pay at the time of service.

Baby looking up.In order to help keep costs down, the charges for office visits are usually paid for at the time of the visit. Patients with insurance are requested to bring insurance forms with them at the time of their visit. If you belong to a managed care plan or HMO, please check with our office manager to see if we participate. Many insurance plans are accepted, and we will be happy to discuss any and all fees with you.

For your convenience, Visa and MasterCard are accepted. Financing is also available.
 

Insurance
The receipt that is given to you after your visit has been designed to be used for insurance purposes. We will help you in whatever way we can, but you can help us at the same time to serve you better if you make sure that your particular insurance policies cover the services for which you want to file claims. Your insurance is generally a contract between you and your insurance company. Everyone's coverage is different, and some services may not be covered by your policy. You are responsible for your bills regardless of what your insurance pays. Payment for all surgical procedures can be discussed with the office manager.
